Important Program Guidelines & Patient Information
To ensure absolute transparency and the best possible experience for our patients, please review the following compliant terms regarding this program:
- 1. Independent Merchant Offer: This program is a private promotional offer funded and managed exclusively by icónica dental group. It is not a direct corporate benefit, insurance product, or official partnership offered by the American Express Company. All fulfillment is the sole responsibility of icónica dental group.
- 2. Insurance Directives: To maintain strict regulatory compliance, these program benefits cannot be combined with any private dental insurance plan (PPO, HMO, or otherwise). Patients must choose to either utilize their standard dental insurance benefits OR the independent icónica Amex benefits for a given visit. These benefits are specifically ideal for patients who are out-of-network, uninsured, or electing for advanced cosmetic and comprehensive reconstructive procedures not typically covered by traditional insurance.
- 3. Non-Transferable: Credits and benefits (such as the $250 restorative credit) have no cash value, are not redeemable for cash, and are completely non-transferable. Credits must be applied strictly to active treatment rendered at icónica dental group.
- 4. Calendar Year Reset: Benefits reset annually on January 1st and do not roll over. Unused benefits from the previous year are naturally forfeited.
